Every Path is Different — Let’s Walk Yours Together

Every individual and family experiences the end-of-life journey uniquely. That's why my services are flexible and personalized, designed to meet you where you are and support you through each step.

🌿 Advance Care Directive Guidance

Making medical decisions for yourself or a loved one can be overwhelming — especially when it’s unexpected. No matter your age or health status, having an Advance Directive in place is one of the most important decisions you can make.

Through compassionate, step-by-step guidance, I’ll help you complete your Advance Directive and Durable Power of Attorney — documents that clearly outline your healthcare wishes if you're ever unable to speak for yourself. You’ll choose the people you trust to carry out those decisions, and I’ll help ensure your wishes are documented, communicated, and respected by your end-of-life care team.

This isn’t just paperwork. It’s peace of mind — for you and the people who care about you most.

✍️ What This Session Looks Like

This isn’t a legal lecture or a one-size-fits-all form. It’s a grounded, human conversation about what matters to you — and how you want to be cared for if you ever can’t speak for yourself.

Together, we’ll explore:

  • What Advance Directives and Powers of Attorney actually do

  • How to choose the right healthcare decision-maker

  • Your preferences around CPR, ventilators, feeding tubes, and comfort care

  • Your values, beliefs, and what “quality of life” means to you

  • Optional legacy messages or guidance for your loved ones

Why Not Just Use the Free Forms?

Yes — most states offer free Advance Directive forms online. And for some people, that’s a helpful first step. But having the form isn’t the same as having a plan that’s clear, meaningful, and confidently communicated.

  • Here’s what you get when we work together:

    • Clarity you can trust – I walk you through each section so you’re not guessing or checking boxes you don’t fully understand.

    • Real conversation – This is about more than medical terms. It’s about your values, your comfort, and the kind of care you want.

    • Support in choosing the right person – We’ll talk through who should speak for you and how to have that conversation with them.

    • Tools for reflection and sharing – I provide optional worksheets and gentle prompts to help you explore your decisions and communicate them with others.

    • Room to grow – I offer discounted yearly reviews or a personal update sheet so your directive can grow with you — because life changes.

“The forms are free. The guidance, clarity, and peace of mind are what you’re really investing in.”

📄 What You’ll Walk Away With

This process goes beyond paperwork. When we’re done, you’ll walk away with:

  • Clarity around your personal values, medical preferences, and end-of-life priorities

  • Confidence knowing your choices have been carefully considered and clearly recorded

  • Peace of mind that the people you trust will be equipped to act on your behalf

  • Supportive tools and prompts to help you reflect, prepare, and share your plan with others

  • A sense of empowerment — not fear — about what lies ahead

FAQs for Advance Care Directive

  • ❓Do I need a lawyer to complete an Advance Directive?

No. In most states, you can legally complete your Advance Directive without a lawyer. I’ll guide you through the process step by step to make sure it’s clear, complete, and reflects your values.

  • ❓Is this a legal document?

Yes — the Advance Directive and Healthcare Power of Attorney forms we complete are legally recognized in your state. I’m not a lawyer and don’t offer legal advice, but I help you understand the documents and how to fill them out correctly.

  • ❓Can I update my Advance Directive later?

Absolutely. Your preferences may change — and your documents can too. I offer optional annual check-ins and update sessions to make sure your plan grows with you.

  • ❓What if I don’t know what I want yet?

That’s exactly why I’m here. You don’t need all the answers. We’ll walk through the options together, talk through what matters most to you, and make decisions at a pace that feels right.

  • ❓What’s the difference between an Advance Directive and a Living Will?

A Living Will is one type of Advance Directive. It usually covers medical preferences like life support, pain management, and end-of-life interventions. An Advance Directive may also include naming a Healthcare Power of Attorney — the person who will speak for you if you can’t speak for yourself.

  • ❓Do doctors actually follow these documents?

Yes — when they’re clear, current, and legally valid. That’s why this session matters. We’ll make sure your choices are documented in a way that healthcare providers and your loved ones can understand and honor when the time comes.
